Personalized Treatment Programs for Long-Term Recovery
Assessment
Intake
During Intake we review each client for short/long-term placements, including psychiatric/medical history and previous progress notes/treatment plans. The G.L.O.M. team develops a Needs and Service plan based on this information, recording baseline data on urgent issues. The Placement Agency may provide suggestions. Program staff observe and record using established data collection systems to evaluate effectiveness and revise the program as needed.
Client Placement
Programs are based on positive reinforcers, weekly incentives based on skill level and preferences using our roadmap to independence. Our individualized programs aim to integrate our clients back into society or reunite them with family.
